29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:  a) 4th tier of the scaffold-  Employees were exposed to a fall of approximately 24 ft while working on a tubular welded scaffold that was missing a guardrail; on or about 5/9/18.
