29 CFR 1926.502(d)(6)(i): A snaphook not designed for directly engage to webbing was engaged directly to webbing Location: a) On the fourth floor balcony on the southwest side of the building: On or about 05/10/18, an employee was installing aluminum panel on a balcony, and engaged his harness lanyard snaphook onto the harness lanyard webbing by wrapping the lanyard around a tubular welded scaffold component. The snaphook is not designed to directly engage onto the webbing. The employee was expose to fall hazard.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person. Location: a) On the fourth floor balcony on the southwest side of the building: On or about 05/10/18, an employee was installing aluminum panel on a balcony, and engaged his harness lanyard snaphook onto the harness lanyard webbing by wrapping the lanyard around a tubular welded scaffold component. The tubular scaffold component as an anchorage is not capable of supporting at least 5,000 pound per employee attached nor was it designed to be use as anchorage. The employee was expose to fall hazard.
