This condition exposed employees to pinch-point and caught-in hazards.    (b)Hyland Machining at 222 South West 4th Street in Sioux Falls, SD 57042: On and about and at times prior to May 15, 2018, the employer did not ensure employees were protected from pinch-point and caught-in hazards in that the stock side panel of the HWACHEON Hi-ECO31AA was not installed exposing, including but not limited to, components of the drive shaft, spindle and the bar stock being feed to the CNC. This condition exposed employees to pinch-point and caught-in hazards.    (c)Hyland Machining at 222 South West 4th Street in Sioux Falls, SD 57042: On and about and at times prior to May 15, 2018, the employer did not ensure employees were protected from pinch-point and caught-in hazards in that the CNC Enhancement Autobar 400 bar stock feeder did not have the panel covering the motor and drive system components installed.  This condition exposed employees to pinch-point and caught-in hazards.    (d)Hyland Machining at 222 South West 4th Street in Sioux Falls, SD 57042: On and about and at times prior to May 15, 2018, the employer did not ensure employees were protected from struck-by and/or caught-in hazards while operating the Yama Seiki AV-1000 in that the employer did not ensure door interlocks were engaged. This condition exposed employees to struck-by and/or caught-in hazards.    (e)Hyland Machining at 222 South West 4th Street in Sioux Falls, SD 57042: On and about and at times prior to May 15, 2018, the employer did not ensure employees were protected from struck-by and/or caught-in hazards while operating the Goodway GLS-200 in that the employer did not ensure door interlocks were engaged. This condition exposed employees to struck-by and/or caught-in hazards.    (i)Hyland Machining at 222 South West 4th Street in Sioux Falls, SD 57042: On and about and at times prior to May 15, 2018, the employer did not ensure employees were protected from struck-by and/or  caught-in hazards while operating the Mori Seiki SL-20 in that the employer did not ensure door interlocks were engaged.
