On or about May 15, 2018, at 4208 Bluebonnet Blvd, Baton Rouge, LA, the employer did not provide fall protection for the employees to use while working on the roof at elevations of 10 feet and above.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about May 15, 2018, at 4208 Bluebonnet Blvd, Baton Rouge, LA, the employer did not ensure the ladder was rextended a minimum of 36 inches above the landing before the employees made use of it to access the roof.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired. On or about May 15, 2018, at 4208 Bluebonnet Blvd, Baton Rouge, LA, the employer did not ensure that a portable ladder with a structural defect was marked or tagged and removed from service. Employees used the ladder with a missing/broken cleat to access the roof of a house at an elevation of approximately 10ft.
