29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system, or on roofs 50-feet (15.25 m) or less in width, the use of a safety monitoring system alone [i.e. without the warning line system] was permitted:      a) Job site, 7670 McKnight Road, Pittsburgh, PA: On or about May 16, 2018, during roofing activities on the lower middle roof section, the employer did not ensure that appropriate fall protection systems/devices were in use while working outside of a flag line warning line system, exposing an employee to a fall hazard of up to approximately 15 feet.     b) Job site, 7670 McKnight Road, Pittsburgh, PA: On or about May 16, 2018, during roofing activities on an upper roof section where the roof hatch and roofing materials were located, the employer did not ensure that appropriate fall protection systems/devices were in use, exposing employees to a fall hazard of up to approximately 20 feet.

29 CFR 1926.502(f)(1)(i): When mechanical equipment was not being used, the warning line was not erected at least 6 feet (1.8 m) from the roof edge:       a) Job site, 7670 McKnight Road, Pittsburgh, PA: On or about May 16, 2018, during roofing activities on the lower middle roof section, the employer did not ensure that flag line warning line systems were set up at an appropriate distance from the edge of the roof in that it was determined that they were set up approximately 4 to 5 feet from the edge of the roof where a fall hazard of approximately 15 feet existed.

29 CFR 1926.502(f)(2)(ii): The rope, wire, or chain was not rigged and supported in such a way that its lowest point (including sag) was no less than 34 inches (.9 m) from the walking/working surface and its highest point is no more than 39 inches (1.0 m) from the walking/working surface:   a) Job site, 7670 McKnight Road, Pittsburgh, PA: On or about May 16, 2018, during roofing activities on the lower middle roof section, the employer did not ensure that flag line warning line systems were at an appropriate horizontal height in that it was determined that the tops of the flags were no higher than approximately 30 inches from the roof.
