(a) Hino Plant, Mineral Wells: On May 16, 2018 and times prior, an employee was working in a trench that had excavated material piled along the edge and was not protected to prevent the material from falling into the excavation.

29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions: (a) Hino Plant, Mineral Wells: On May 16, 2018 and times prior, an employee worked in the trench two days installing a water line for sprinkler system. Daily inspections of the trench were not made prior to the start of work each day.
