29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    Instances 1:   Approximately 2 inch rubber belts were not guarded to protect an operator adjusting boxes above the belt system of an Arc /Doyle Counter-Ejector from ingoing nip points on the belts.      Instances 2:   An employee was exposed to ingoing nip point hazards from five rubber belts that were not guarded while an employee was cleaning up boxes from under an Arc/Doyle Counter-Ejector.
