29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level.    On 5/15/2018, employees from Dependable Construction II LLC were installing siding onto a residential building located on lot 3 Hannahs Mill Drive, Lake St. Louis, MO. The employees were on a Werner  pump jack scaffold system approximately 10-15 feet above the ground. The employees were not protected from falling to the lower level.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. Guardrail systems were not installed before the scaffold was released for use by employees other than erection/dismantling crews.    On 5/15/2018, employees from Dependable Construction II LLC were installing Siding onto a residential building located at lot 3 Hannahs Mill Drive, Lake St. Louis, MO.   The Werner Pump Jack Scaffold employees were working from did not have guardrail systems installed along all open sides and ends of platforms.

On 5/15/2018, employees from Dependable Construction II LLC were installing Siding onto a residential building located at lot 3 Hannahs Mill Drive, Lake St. Louis, MO.   The Werner Pump Jack Scaffold employees were working from was not secured to the structure by rigid triangular bracing or equivalent means, at the bottom. The support poles did not have a physical connection preventing horizontal movement.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.     On 5/15/2018, employees from Dependable Construction II LLC were installing Siding onto a residential building located at lot 3 Hannahs Mill Drive, Lake St. Louis, MO.   The ladder used to access the roof was not extended three feet beyond the landing or edge of the roof.
