29 CFR 1910.176(a):  Where mechanical handling equipment is used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made.  Permanent aisles and passageways were not appropriately marked:  a) At the warehouse, employees were exposed to struck-by hazards while working near forklift operation without proper aisle and passageway markings.
