29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective  JOB SITE: 30 Hat Trick Lane, Wright City, MO  63390, on or about 5/17/2018, the employees were observed operating electrical power hand tools, actively assembling metal trusses in the production area of the facility.  The electrical power hand tools were attached to extension cords with missing ground prongs.  The employees were exposed to potential electrical hazards.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  JOB SITE: 30 Hat Trick Lane, Wright City, MO  63390, on or about 5/17/2018, in the Production area, flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws.  The workers were exposed to electrical hazards.    The following extension cords were damaged:    1)      Inside of the Production area, an extension cord was used to power the Triton Band Saw.  2)      Inside of the Production area, located next to the employees break area, The extension cord connected to the Global 30" Pedestal Fan.
