29 CFR 1904.41(a)(1): The employer had not electronically submitted information from their recordkeeping forms required under this part once a year and no later than the date listed in paragraph (c) of this section:  1) On May 16, 2018, and at prior times, the employer failed during calendar year 2017 to electronically submit information from their 2016 OSHA form 300A or equivalent by December 15, 2017.

29 CFR 1926.1101(h)(3)(iv)(A): The employer did not provide employees with tight-fitting powered air-purifying respirator or a full facepiece, supplied-air respirator operated in the pressure-demand mode and equipped with either HEPA egress cartridges or an auxiliary positive-pressure, self-contained breathing apparatus (SCBA) whenever the employees are in a regulated area performing Class I asbestos work for which a negative exposure assessment is not available and the exposure assessment indicates that the exposure level will be at or below 1 f/cc as an 8-hour time-weighted average (TWA):  1) On May 18, 2018, employees were not provided with powered air-purifying respirators or full facepiece supplied-air respirators when conducting Class I asbestos work where there was no negative exposure assessment.
