The employer shall provide a medical evaluation to determine the employee's ability to use a respirator, before the employee is fit tested or required to use the respirator in the workplace. The employer may discontinue an employee's medical evaluations when the employee is no longer required to use a respirator.  a. At the facility, an employee used a 3M 6000 respirator with a 2091 P100 filters and the employer did not ensure that a medical evaluation was done to determine the employee's ability to use the respirator and perform work.

29 CFR 1910.134(f)(2): The employer shall ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter.  a. At the facility, an employee used a 3M 6000 respirator with 2091 P100 filters to perform work and the employer had not first ensured that the employee was fit-tested to ensure a good respirator face piece to face seal.

29 CFR 1910.157(e)(2): Portable extinguishers or hose used in lieu thereof under paragraph (d)(3) of this section shall be visually inspected monthly.    a) On the work floor, three of the company's four fire extinguishers had last been inspected in April 2015 and the fourth fire extinguisher was missing from its placard location. One of the fire extinguishers had been used and not refilled. The portable fire extinguishers had not been inspected monthly.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets.  a. At the facility, employees who used chemicals such as, but not limited to, EA Black-70, propane, argon, acetylene, and LP Gas had not received information and training exposing them to inhalation and contact hazards.
