29 CFR 1910.67(c)(2)(v): (v) A personal fall arrest or travel restraint system that meets the requirements in 29 CFR part 1910, subpart I, shall be worn and attached to the boom or basket when working from an aerial lift.  Worksite: A Fall arrest system was not used by an employee working from a bucket truck. The bucket snapped at the boom and the employee fell 25 feet to the ground below.
