29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  On May 21, 2018, there were no frequent and regular inspections being performed by a competent person in that employees were performing roofing work at fall heights greater than 19 feet above the ground below without a fall protection system; employees were accessing the roof by using a 10 foot portable extension ladder that did not extend at least 3 feet above the upper landing surface; and the employers designated Competent Person/Foreman was observed working alongside other exposed employees in plain view.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.   The employer did not provide a training program for employees that were observed performing roofing work on a residential roof structure while not utilizing a fall protection system and exposed to fall heights greater than 19 feet above the ground below.
