An employee designated as a Laminator was utilizing a MSA Advantage Model 200 LS half-mask respirator with P100/OV cartridges in Lamination and was not medically evaluated prior to the use of the respirator.   c) In the facility, on or about May 22, 2018 - The employer did not ensure that employees required to wear tight-fitting respirators were provide a medical evaluation to determine their ability to use a respirator in the workplace.  An employee designated as a Painter was utilizing a MSA half-mask respirator with P100/OV cartridges in Painting and was not medically evaluated prior to the use of the respirator.
