29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  Rail Yard: On or about 5/3/18, and at times prior, the employer exposed employees to chemical hazards, in that plumbed eyewash stations for quick drenching of the eyes were not provided to employees while working with corrosive hazardous material such as 50% Sodium Hydroxide (NaOH) or acetic acid.

29 CFR 1910.134(f)(1): The employer did not ensure that employee(s) required to use a tight-fitting facepiece respirator passed the appropriate qualitative fit test (QLFT) or quantitative fit test (QNFT):   (a) Rail Yard- On or about 5/3/18, and at times prior, the employer exposed employees to upper respiratory irritation, in that FIT test were not provided to employees prior to wearing 3M tight fitting half-face respirators.
