29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to their environment to control or eliminate any hazards or other exposure to illness or injury:    a) Roth Trucking, Inc. at 28346-28410 SD 34, Fort Pierre, SD  57532:  On or about and at times prior to May 22, 2018, the employer did not ensure truck drivers were instructed in the recognition and avoidance of unsafe conditions.  The employer did not effectively instruct drivers in identifying conditions related to struck-by, caught-in-between, and crushing hazards associated with the movement of heavy construction-related equipment.  This condition exposed an employee to struck-by, caught-in-between and crushing injuries.
