On or about 05/17/2018, at 2913 Fable St. - employees finishing the walls with stucco were exposed to a 19 feet 11 inch, a 13 foot 6 inch and a 7 feet 2 inch fall hazards, in that, the scaffold was not fully planked.

On or about 05/17/2018, 2913 Fable St. - employees working from the scaffold lower levels and from the ground were exposed to head injured created by struck by hazard from falling objects such as tools, in that, hard hats were not used.
