Employees in the delivery warehouse were exposed to crushing, and struck-by hazards. . Employees were operating forklifts, including but not limited to the Caterpillar 2P6000 serial number AT13F31092, based off  training and certification from the Cassville Missouri location. Employer did not develop a site specific forklift program
