29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:      a)  On May 29, 2018, an extension ladder used by employees to gain access to the ladder jack scaffold platform was not long enough to have it's side rails extend at least 3 feet above the platform.      Bismark Roofing, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1279434, citation number 1, item number 2 and was affirmed as a final order on February 22, 2018, with respect to a workplace located at 338 Woodstock, Kenilworth, Illinois 60043.     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end:      a)  On May 29, 2018, employees were working from a ladder jack scaffold platform which was extended approximately four feet over the end support.       In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person:     a)  On May 29, 2018, the employer did not ensure that the anchorage used by employees as an attachment point for personal fall arrest system was capable of supporting at least 5,000 pound per employee attached to it.      No abatement certification or documentation is required for this item.
