Wristlets may be used in lieu of the chest or full body harness if the employer can demonstrate that the use of a chest or full body harness is infeasible or creates a greater hazard and that the use of wristlets is the safest and most effective alternative. a. On or about May 25, 2018, an employee cleaning the inside of a semi-truck tanker was pronounced dead following a confined space rescue. The employee was found to have suffered chemical asphyxia by vitiated atmosphere, hydrogen sulfide and hydrogen cyanide gases. At the time of the incident the entrant had entered the permit-required confined space without a full body harness.

The employee was found to have suffered chemical asphyxia by vitiated atmosphere, hydrogen sulfide and hydrogen cyanide gases. Where the hazardous atmosphere had not been eliminated prior to entry, a permit required confined space entry was not performed. Employees had not been designated and trained to perform the tasks of confined space entrant, attendant, and entry supervisor in that the measures required to complete a permit-required confined space permit were not performed, the permit was not signed by an entry supervisor and an attendant was not positioned to communicate with the entrant and to monitor the entrant's activities and alert the entrant of the need to evacuate the space or to summon rescue services.
