29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  Location - 15 Wallis St. Peabody, MA  On or about 30 May, 2018, the employer didn't ensure employees working at heights approximately between 9 ft. to 27 ft. were protected from falls to a lower level by fully planking the platform and installing guardrails.
