Employees change the dies used to cut products such as doorknob hanger advertisements.  There is no procedure for controlling stored energy on the machine during this process.      (b)  315 S. 12th Street, Montrose CO 81401:  On or about May 23, 2018, and at times prior, employees were exposed to struck by or caught in hazards due to lack of an energy control procedure for the Miehle V50 Vertical Press.  Employees change the dies used to cut products such as doorknob hanger advertisements.  There is no procedure for controlling stored energy on the machine during this process. (c)  315 S. 12th Street, Montrose CO 81401:  On or about May 23, 2018, and at times prior, employees were exposed to struck by or caught in hazards due to unguarded in-running nip points on the Ryobi 3200 CD Envelope Stamper.  Employees operate the envelope stamper to stamp custom addresses on envelopes.  During this process, the operating employee is standing next to unguarded rollers which create an in-running nip point hazard.    (d) 315 S. 12th Street, Montrose CO 81401:  On or about May 23, 2018, and at times prior, employees were exposed to struck by or caught in hazards due to unguarded hollow drill bits on the Challenge EH-3C Paper Drill.  Employees use a foot pedal to operate the paper drill.  During this process, the employee is placing their fingers in the vicinity of the unguarded drill bit and the clamp used to hold the paper in place.    (e)  315 S. 12th Street, Montrose CO 81401:  On or about May 23, 2018, and at times prior, employees were exposed to crushing or puncture hazards due to unguarded moving parts on the Interlake 76 Stitcher.  Employees must hold the paper in place while the Stitcher cuts staples from steel wire and staples the paper.  This puts the hands of the operator near unguarded moving parts.
