29 CFR 1926.1412(b)(4): Equipment must not be used until an inspection under this paragraph demonstrates that the repair/adjustment meets the requirements of paragraph (b)(1)(i) of this section:  On or about May 30, 2018, employees were exposed to struck-by hazards while using a modified crane (General/ GR1) prior to having the crane being inspected. A load test (performed by a third party) was discontinued due to unsafe conditions.

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace.    On or about May 30, 2018, employees were exposed to respiratory health hazards because the employer did not ensure medical evaluations were conducted prior to allowing employees to use respirators while handling chemicals.
