29 CFR 1926.453(b)(2)(v):  A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift.  a. 4828 Tonnele Ave North Bergen, NJ  Employee was exposed to fall hazards of more than 18 feet to the ground while on an aerial lift without any means of fall arrest system such as a harness and lanyard attached to the basket. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  a. 4828 Tonnele Ave North Bergen, NJ  Employee(s) were observed performing construction activities on the walking / working surface in close proximity to an unprotected edge without any means of fall protection, thereby exposing workers to an approximate 18 ft fall hazard.  Violation occurred on or about 05/31/2018
