29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements of subpart I of this part were not worn or attached to the boom or basket when working from an aerial lift. On or about May 31, 2018, two employees were working from inside the basket of a sign installation truck and neither of the employees was wearing fall protection, exposing employees to a fall hazard up to approximately 18 feet.

29 CFR 1910.30(a)(3)(iv): The employer did not train each employee on the correct use of personal fall protection systems and equipment, including, but not limited to, proper hook-up, anchoring, and tie-off techniques, and methods of equipment inspection and storage, as specified by the manufacturer: On or about May 31, 2018, and at times prior thereto, two employees were installing a sign on a commercial property under construction from the basket on the sign truck and neither had received training on hazards associated with falls, exposing employees to a fall hazard up to approximately 18 feet.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On or about May 31, 2018, and at times prior thereto, two employees were not protected by eye or face protection while using a drills and grinders to install a sign on the side of the commercial property, exposing employees to the hazard of being struck by flying debris.
