29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use.  On or about May 31, 2018, the eyewash bottles provided in the company facility, located at 1700 Rangeline Road, Eagle River, WI, were not suitable since they were not capable of providing 15 minutes of continuous flushing when working with corrosive materials such as Mold Science Technologies On & Gone.

29 CFR 1910.1200(f)(6)(ii): Product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, will provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.  On or about May 31, 2018, secondary containers did not contain information relating to the hazards of the chemicals used in the container and were being used over multiple shifts. Containers missing labels included a bottle labeled "Zero Cleaner" and large spray canisters labeled "Benefact".

29 CFR 1910.1200(g)(8): The employer shall maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and shall ensure that they are readily accessible during each work shift to employees when they are in their work area(s). (Electronic access and other alternatives to maintaining paper copies of the safety data sheets are permitted as long as no barriers to immediate employee access in each workplace are created by such options.)  On or about May 31, 2018, employees working out of the Eagle River location of Dringoli, Inc, dba PuroClean were not provided access to safety data sheets when working with cleaners such as Mold Science Technologies On & Gone, 1st AYD "Super Bowl" Heavy Duty Butyl Degreaser and Benefect Botanical Disinfectant. The SDSs were kept electronically but were only accessible to the Supervisor of the Eagle River location who is not always available to the employees.

29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.  On or about May 31, 2018, a certified hazard assessment had not been conducted to assess that appropriate Personal Protective Equipment (PPE) had been selected for all possible work including mold remediation, renovation and disaster recovery.
