29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury.    Location - 160 Cambridge Street, Burlington, MA.    On or about May 31, 2018, employees were not adequately instructed in the recognition and avoidance of fall hazards while working on a commercial roof and an aerial lift.     Instance one: Two employees were working/walking on the roof with no fall protection system in place to prevent them from falling to lower level. Roof height measurement provided by employer reached up to twenty five feet above lower level.      Instance two: An employee was working in an aerial lift installing siding on a commercial building. The employer provided the height the employee was working at which was seventeen and half feet above lower level.
