29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.   On or about June 1, 2018, employees working from a frame scaffold were not provided with enough planks to safely plank the entire work surface, exposing employees to a fall hazard of approximately 19 feet.   Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.451(c)(1)(ii): The top guy, tie, or brace of completed scaffolds were placed further than the 4:1 height from the top.  Such guys, ties and/or braces were not installed at each end of the scaffold and/or at horizontal intervals not to exceed 30 feet (9.1 m):   On or about June 1, 2018, employees were exposed to fall hazards while working from a frame scaffold with a ratio of 5:1 that was not protected from scaffold collapse by guying or tying the scaffold.      Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.   On or about June 1, 2018, employees working from a frame scaffold were not provided with a safe means of access, exposing employees to a fall hazard and other associated other injuries.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
