29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems:  On or about May 30, 2018, an employee entered an excavation between the soil and the forms for a wall to be poured and a daily inspection of the excavation, adjacent area and the sloping system was not made by a competent person for evidence of a situation that could result in a cave-in hazard.

29 CFR 1926.652(b): The slopes and configurations of sloping and benching systems were not in accordance with the requirement options of 29 CFR 1926.652(b)(1), (b)(2), (b)(3), or (b)(4):  On or about May 30, 2018, an employee entered an excavation between the soil and the forms for a wall to be poured and the sloping system was not dug in accordance with the options in the standard to protect employees from cave-in hazards.
