29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: (a) Jobsite: On or about June 1, 2018, the employees were using a non approved (for damp and wet locations) quad receptacle box at the end of an extension cord. This extension cord and electrical box was being used outside.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: (a) Jobsite: On or about June 1, 2018, the grounding pin to the extension card that the employees were using had its grounding pin pulled. In addition, the insulation to this extension cord was damaged in at least three different places.
