29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:.    Location - 160 Cambridge Street, Burlington, MA.     On or about May 31, 2018, two employees were walking/working on a roof at a height greater than seventeen feet above lower level. Employees were installing new shingles on a commercial building.

29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s).    Location - 160 Cambridge Street, Burlington, MA.    On or about May 31, 2018, employer did not conduct frequent or regular inspection of the job site that would of identified two employees working/walking on the roof with no fall protection system in place to prevent them from falling. The two employees were working at a eight greater than seventeen feet above lower level.
