29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  Employees engaged in grandstand construction at the 17th hole were exposed to fall hazards in that no type of fall protection was provided during the installation of decking. Employees were exposed to a fall hazard of approximately eight (8) feet.

29 CFR 1904.40(a): The employer did not provide an authorized government representative the records within the four business hours.  On 04 June 2018, a request was made via email for the OSHA 300 logs, another request was made on 10 July 2018 for the same logs, a confirmation portion of the requested information was received on 11 June 2018 via email. However, the 300/300As were not received. The logs had not been provided as of 25 July 2018.
