29 CFR 1926.652(a)(1): Employees in excavations were not protected from cave-ins by an adequate protective system designed in accordance with paragraphs (b) sloping and benching or (c) support/shield/protective systems of this section: The employer failed to provide cave-in protection such as sloping, benching, or use of a trench box when employees were working in an excavation that was approximately ten (10) feet in depth, two (2) feet wide, twenty-eight (28) feet in length, and comprised of material consistent with that of a Type B soil.
