Deficiencies noted included limited numbers of anchor points being installed which created a situation where employees would have to walk long distances away from anchor points and be exposed to falls off of the roof in the process and not ensuring the employees had at least two tie off devices in order to ensure 100% fall protection was in use when transitioning from one rope to another to perform work.
