29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:      a) Job site, 214 Mansfield Avenue, Carnegie, PA: On or about June 4, 2018, during roofing activities, the employer did not ensure that a safe means of getting on/off the roof existed in that the  extension ladder was not extended at least 3 feet above the landing and no grab bar was provided.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:      a) Job site, 214 Mansfield Avenue, Carnegie, PA: On or about June 4, 2018, during roofing activities, the employer did not ensure that employees were protected by fall protection systems/devices where fall hazards of up to at least approximately 20 feet existed.    Pitt Contracting Service, LLC was previously cited for a violation of this occupational safety and health standard or its' equivalent standard, 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1147369, citation number 1, item 2a, and was affirmed as a final order on June 13, 2016, with respect to a workplace located at 131 South College Street, Washington, PA.

29 CFR 1926.503(a)(2)(iii): The employer did not train employees working at heights in the proper use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used:   a) Job site, 214 Mansfield Avenue, Carnegie, PA: On or about June 4, 2018, during roofing activities, the employer did not ensure that proper fall protection training was provided for employees where fall hazards of up to at least approximately 20 feet existed. Deficiencies noted included limited numbers of anchor points being installed which created a situation where employees would have to walk long distances away from anchor points and be exposed to falls off of the roof in the process and not ensuring the employees had at least two tie off devices in order to ensure 100% fall protection was in use when transitioning from one rope to another to perform work.
