29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  On Friday 11 May, 2018 at approximately 4:45 PM, an employee was operating a Polaris when it rolled over onto the employee resulting in a hospitalization. The hospitalization was not reported within 24 hours to the local Area Office of the Occupational Safety and Health Administration. The Austin Area Office was notified on 4 June 2018 at approximately 11:30 AM.
