29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest system.    (a)  Employees conducting roofing activities on a roof with a 3:12 pitch were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 22' fall hazard.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    (a)  The employer did not ensure that employees exposed to potential fall hazards had been adequately trained to recognize and minimize the fall hazards associated with their work and on the limitations of the fall protection system in use. This exposed employees to a fall hazard of approximately 22' fall hazard.
