29 CFR 1926.1053(b)(6): Ladders  were not used only on stable and level surfaces and were not secured to prevent accidental displacement:      a) Job site, 30 South 6th Street, Pittsburgh, PA: On or about June 5, 2018, during carpentry activities, the employer did not ensure that the ladder used for moving between ground level and the roof at the front of the building was properly set up. It was used on an unstable surface, had a rock or a brick placed under one of it's feet and was not secured at the top to prevent accidental displacement, exposing employees to fall hazards of up to approximately 20 feet.

29 CFR 1926.1053(b)(15): Ladders were not inspected by a competent person for visible defects on a periodic basis and after any occurrence that could affect their safe use:   a) Job site, 30 South 6th Street, Pittsburgh, PA: On or about June 5, 2018, during carpentry activities, the employer did not ensure that the ladder used for moving between ground level and the roof at the front of the building was inspected by a competent person. Deficiencies noted during the inspection included, but were not limited to, bent/damaged rungs, part of a siderail was missing and one of the lower legs was bent/damaged.
