29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about June 5, 2018, employees working inside of the flat storage building were exposed to amputations from contact with an exposed rotating floor auger and machine guarding was not provided.

29 CFR 1910.272(h)(2)(i): Whenever an employee walks or stands on or in stored grain or grain products of a depth which poses an engulfment hazard, all equipment which presents a danger to that employee (such as an auger or other grain transport equipment) was not deenergized, and was not disconnected, locked-out and tagged, blocked-off, or otherwise prevented from operating by other equally effective means or methods:    On or about June 5, 2018 and at times prior, employees working inside of the flat storage building were exposed to injuries when from contact with an exposed rotating floor auger and the auger was not deenergized while working on the ground level of the flat storage building.
