On or about November 2, 2018 the employer failed to submit the required abatement documents demonstrating the violated standard had been abated.

29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not  independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee.    On or about June 6, 2018 at the construction site employees were exposed to fall hazards while using anchorage points that did not support at least 5,000 pounds per employee.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used:  On or about June 6, 2018 at the construction site where an employee was exposed to a fall hazard of more than 6 feet while working from a roof without being trained by a competent person on the use of a personal fall arrest system.
