NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.134(k): The employer did not provide comprehensive and understandable training annually to employees who were required to use respirators and did not provide the basic information on respirators in Appendix D of this section to employees who wore respirators when not required by this section or by the employer to do so:  a.) Facility:  Employee(s) provided 3M brand dust masks for voluntary use were not provided the information in Appendix D, as observed on 6/7/2018.  b.) Production floor: Employees required to wear 3M 6001 half faced air purifying respirators with ov cartridges were not provided comprehensive and understandable training annually as observed on 6/7/2018.  NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
