29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  At a worksite located at 615 NE Lone Hill Lane in Lee's Summit, MO employees were exposed to a fall hazards. Employee were exposed to a fall of approximately 10 feet to the ground below in that a opening in the floor had not been covered or provided with any means of protective systems to preventing a fall.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   At a residential construction site located at 615 NE Lone Hill Lane in Lee's Summit, MO: Employees were exposed to fall hazards. The employer failed to protected employees from fall related hazards in that employees were observed working at a height of 10.7 feet without the use of any protective systems.
