29 CFR 1910.333(c)(3)(iii)(A): Vehicles or mechanical equipment capable of having parts of their structures elevated near energized overhead lines were not operated so that a clearance of 10 feet (305 cm) plus 4 inches (10 cm) for every 10 kV over 50 kV:  On or about May 17, 2018, at the Electrical Energy Inc., Joppa Power Station, in the Ash Pond:  An employee working for the Jesco Industrial Services Company was operating a Supersucker Vacuum Truck, #301052, elevating the tank.   The truck was within 4.5 feet from the 345 kV transmission lines when the lines arced to the truck.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye:  On or about May 17, 2018, an employee was injured while working at the job site at Joppa Il, resulting in the hospitalization of that employee.  A report of the event was not made to OSHA.
