29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: (a) New residential construction located on Lot #14, South gable side: On or about 05/31/2018, and at times prior, the employer exposed employee to fall hazards of approximately 19-feet to the lower level, in that the employee engaged in roofing activities was not provided with a compliant work platform.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: (a) New residential construction located on Lot #14, South gable side: On or about 05/31/2018, and at times prior, the employer exposed an employee to fall hazards of approximately 19-feet to the lower level, in that the employer failed to secure the makeshift work platform to the forks to prevent the platform from moving during use.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: (a) New residential construction located on Lot #14, South gable side: On or about 05/31/2018, and at times prior, the employer exposed an employee to fall hazards of approximately 19-feet from the lower level, in that an adequate fall protection system was not provided to an employee engaged in roof work on a makeshift work platform attached to an all-terrain forklift truck.
