29 CFR 1926.1428(b): If subsequent actions by the signal person indicate that the individual does not meet the Qualification Requirements, the employer must not allow the individual to continue working as a signal person until re-training is provided and a re-assessment is made that confirms that the individual meets the Qualification Requirements:  On or about May 10, 2018,  Location: Roof of Bronx River Media Group Sound Stages new building;  1) Employees working on the roof were exposed to being struck by and exposed to fall hazards up to 30ft high while a structural member (joist) was being placed using a mobile crane and the signal man did not demonstrate having the qualification requirements at the time of the incident. The signal man (on the roof) gave instructions to the crane operator (on the ground level) via radio to cable up, when the connectors were still releasing the cables from the joist and one of the connectors was still sitting on top of the joist. Subsequently, the signal man did not alert the crane operator to stop cabling up when the cable's hook got wrapped up at one end of the joist being placed by the connectors. This caused the joist to slide, falling from the opposite side approximately 12ft down to the second floor deck with one employee (first connector) sitting on it, while the other side of the joist moved up, knocking the other employee (second connector) and causing him to fall approximately 10ft down, to hit a concrete wall, and then to fall approximately 20 ft more to the ground level.
