On or about June 11, 2018, three employees were working in an excavation that was approximately 7 feet deep without a protective system in place, exposing the employees to a cave-in hazard.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  On or about June 11, 2018, employees were utilizing an 8-foot, portable, free-standing ladder that was folded and leaning against the side of the excavation for egress, exposing the employees to a fall hazard.
