29 CFR 1910.334(a)(2)(ii): If there is a defect or evidence of damage that might expose an employee to injury, the defective or damaged item shall be removed from service, and no employee may use it until repairs and tests necessary to render the equipment safe have been made.  a) Meat section: On or about June 3, 2018, and at times prior, the employer exposed employees to electrical shock hazards while using a meat cutting band saw, in that the band saw was connected to an unsecured electrical outlet.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and cables may not be used as a substitute for the fixed wiring of a structure.  a) Meat Section: On or about April 19, 2017, and at times prior, the employer exposed employees to electrical shock and fire hazards while utilizing a weight scale that was connected to an extension cord and tied to the building structure.
