29 CFR 1926.100(a):   Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  The employer is failing to ensure that employees were not exposed to potential head injuries.  This was most recently documented on June 13, 2018 at 419 E. Decatur St. West Point, Nebraska.  The employer failed to ensure that employees working at the ground level were not exposed to debris being thrown from the 3 story roof above into the dumpster.

29 CFR 1926.252(a):   An enclosed chute of wood, or equivalent material, was not used where materials were dropped more than 20 feet to points lying outside the exterior walls of the building(s):  The employer is failing to ensure that employees were not exposed to a struck-by hazard.  This was most recently documented on June 13, 2018 at 419 E. Decatur St. West Point Nebraska.  Debris was being thrown off of the roof into and around a dumpster located 32 feet below while an employee was working below.

29 CFR 1926.501(b)(10):   Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  The employer is failing to ensure that employees were protected from a fall hazard.  This was most recently documented on June 13, 2018 at 419 E. Decatur St. West Point, Nebraska. The employees were working on a low sloped roof with out the use of a safety monitoring system or a warning line as the building was greater than 50 feet in least dimension.  The ground to eave height was greater than 25 feet.

This was most recently documented on June 13, 2018 at 419 E. Decatur St. West Point Nebraska.  The employer failed to train his employees on the use of fall protection when working on a flat roof.  The employees were not aware of the requirements to use a monitor or warning line when working on a flat roof.
