29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice:    On or about June, 14, 2018, the walking/working surfaces at the entrances to the B freezer building and throughout the B freezer aisles and the dock aisle were icy and slippery. Employees operating forklifts, pallet jacks and walking through were sliding and had difficulty stopping.

29 CFR 1910.178(l)(4)(i): Refresher training, including an evaluation of the effectiveness of that training, was not conducted as required by paragraph (l)(4)(ii) to ensure that the operator had the knowledge and skills needed to operate the powered industrial truck safely:  On or about June 14, 2018, powered industrial vehicle operators did not receive refresher training and evaluation post impact incidents with other powered industrial vehicles, stationary barrier post, and striking employees.
