29 CFR 1910.22(a)(2): The floor of each workroom was not maintained in a clean and, to the extent feasible, in a dry condition: (a) Re-Saw 1: On or about June 13, 2018 the floor around the Number 1 Re-Saw had a large quantity of saw dust piled around the saw and into the walk way next to the saw creating a slipping and tripping hazard. (a) Re-Saw 2: On or about June 13, 2018 the floor around the Number 2 Re-Saw had a large quantity of saw dust piled around the saw and into the walk way next to the saw creating a slipping and tripping hazard.

29 CFR 1910.22(c): The employer did not provide, and ensure each employee used, a safe means of access and egress to and from walking-working surfaces: (a) Ballinger Stacker: On or about June 13, 2018 employees were required to step from the guarded operating platform beside the Ballinger Stacker and onto an eleven inch portion of an elevated pallet platform to place stringers on the finished boards. The twenty six inch high partial platform contained free rolling wheels and a large stationary roller that exposed employees to a slipping and tripping hazard.
